Isotretinoin is one of the strongest human teratogens ever identified. A single dose during early pregnancy — often before a missed period, before the patient even knows they're pregnant — is enough to cause what's called retinoic-acid embryopathy. In pregnancies exposed during weeks 3–8, roughly one in three babies has a major malformation:

  • Face & head: missing or malformed ears (microtia / anotia), cleft palate, small jaw (micrognathia)
  • Brain & CNS: hydrocephaly, microcephaly, intellectual disability
  • Heart: conotruncal defects, great-vessel transposition
  • Thymus: thymic aplasia → lifelong immunodeficiency
  • Eye: optic-nerve hypoplasia, microphthalmia

Exposed pregnancies also have much higher rates of spontaneous abortion and stillbirth. These outcomes are not hypothetical — they're documented in every historical cohort, which is exactly why the FDA created iPLEDGE in the first place.

The stricter protocol for women isn't about being paternalistic — it's about making sure nobody starts isotretinoin with a pregnancy that nobody knows about yet, and that the dual-contraception window around therapy is airtight.
